Environmental Health Department <br />CP22 - UST FULL Inspection Report <br />Date: <br />October 29, 2024 <br /> Facility Address: <br /> 7700 MORELAND ST, STOCKTON <br /> Facility Name: <br /> GAVINKO PETROLEUM* <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />This is a repeat violation (Class 2) Violation. <br />CA Code of Regulations 2716(a-e) 2030013 - DO inspections completed at least every 30 days and meet all <br />requirements (TCR 12). <br />OBSERVATION: Designated operator reports were not signed within the required time frame. <br />-The 3/18/2024 & 2/9/2024 designated operator report was NOT signed by the owner in section 6 <br />REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual inspection , the <br />designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours <br />of being provided a signed copy of the report, the owner or operator shall provide a description of each corrective action <br />taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. <br />CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the <br />required time frame in the future. <br />------------------------------------------------------------------------------------------------------------ <br />OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports. <br />The incorrect and missing information included the following from the inspection reports: <br />- 7/9/24 no CERS ID in Section 1 <br />- 1/12/24 & 3/18/2024 & 4/15/24 & 7/9/2024 Section 5 was not complete by owner despite there being compliance <br />issues <br />- 3/18/2024 the name of the DO is not indicated in section 2 <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing <br />and maintenance for the UST system have been completed and shall check all required items on the report . <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all <br />of the required information on the reports. <br />This is a repeat violation (Class 2) Violation. <br /> 56 <br />CA Code of Regulations 2712(b)(1-2) 2030001 - Records of release detection monitoring and maintenance properly <br />maintained (TCR 9d). <br />OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br />- EHD was notified by Elite IV that a cold start was done on 9/12/24 due to a bad NVMEM board, work order was not <br />found on site. <br />- EHD was notified by Elite IV that a 120 Smart relay on 87 was replaced on 7/21/2024, work order was not found on <br />site. <br />REGULATION GUIDANCE: These records shall be maintained on site for at least three years.
