The following is an itemized list of underground storage tank violations that have not been addressed for WAYZ FOOD & LIQUOR WAYZ GAS & LIQUOR as of January 26, 2024. Open violations from December 14, 2023 inspection Violation #208 - Failure to comply with the designated operator inspection requirements. OBSERVATION: The designated operator inspections for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 5/17/2023, next inspection performed on 7/19/2023 (63 days) REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #209 - Failure to maintain a copy of the "Facility Employee Training Certificate" on-site. OBSERVATION: A training record was not observed on site. REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #307 - Failure to keep water out of the secondary containment system (post -Jul 2003). OBSERVATION: Water was found in the transition sump and diesel STP sump. REGULATION GUIDANCE: Secondary containment shall be constructed to prevent any water intrusion into the system by precipitation, infiltration, or surface runoff. CORRECTIVE ACTION: The owner removed approximately 1.5 gallons of liquid from the transition sump and approximately 2 cups of liquid from the diesel STP sump and returned the sensor to its proper location. Immediately contact a properly licensed, trained, and certified contractor to address the water intrusion into the transition sump under permit and inspection of the EHD. Page 4 of 5
