The following is an itemized list of underground storage tank violations that have not <br />been addressed for WAYZ FOOD & LIQUOR WAYZ GAS & LIQUOR as of <br />January 26, 2024. <br />Open violations from December 14, 2023 inspection <br />Violation #208 - Failure to comply with the designated operator inspection requirements. <br />OBSERVATION: The designated operator inspections for the following months were performed more than 30 days <br />after the previous inspection: <br />- Initial inspection on 5/17/2023, next inspection performed on 7/19/2023 (63 days) <br />REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of <br />the previous inspection. <br />CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br />frames. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #209 - Failure to maintain a copy of the "Facility Employee Training Certificate" on-site. <br />OBSERVATION: A training record was not observed on site. <br />REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if <br />after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training <br />Certificate shall contain accurate information for all employees trained. <br />CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br />submit a copy of the updated form to the EHD. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #307 - Failure to keep water out of the secondary containment system (post -Jul 2003). <br />OBSERVATION: Water was found in the transition sump and diesel STP sump. <br />REGULATION GUIDANCE: Secondary containment shall be constructed to prevent any water intrusion into the <br />system by precipitation, infiltration, or surface runoff. <br />CORRECTIVE ACTION: The owner removed approximately 1.5 gallons of liquid from the transition sump and <br />approximately 2 cups of liquid from the diesel STP sump and returned the sensor to its proper location. <br />Immediately contact a properly licensed, trained, and certified contractor to address the water intrusion into the <br />transition sump under permit and inspection of the EHD. <br />Page 4 of 5 <br />
