OBSERVATION: The following were not attached to the designated operator inspection reports: -The 9/22/2023 designated operator report did not have a statement or work order associated to the compliance issues noted by the designated operator(T2 leak alarm 9/17/2023). REGULATION GUIDANCE: The designated operator visual inspection report shall include: -a copy of documentation demonstrating action taken in response to each compliance issue identified during the previous inspection -a copy of the alarm history since the previous inspection -a copy of documentation demonstrating action taken in response to each alarm since the previous inspection -a list of each alarm since the previous inspection for which there is no documentation of the alarm condition and action taken in response CORRECTIVE ACTION: Ensure all required documents are attached to future designated operator inspection reports. This is a repeat violation, Class II. 214 CCR 2712 Failure to comply with UST operating permit conditions. OBSERVATION: The 91 STP sump 205 sensor was replaced with a 208 sensor on 10/24/2023 and a permit was not obtained from the EHD. Notification was provided to EHD on 10/25/2023 but the notification indicated a like-for-like 208 sensor replacement. CERS has listed the sensors at this facility as 205 sensors.A 208 sensor was observed during the inspection today.A 205 to a 208 sensor replacement does not qualify as a like-for-like replacement since these are two difference sensors. REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the 205 to 208 sensor replacement that occurred 10/24/2023 . A"Failure to Obtain a Permit" penalty applies. This is a minor violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 3/13/2024. To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of having them sign.
