HSC 119310, 119311 -Jewelry, Inks, Needles etc approved and used correctly. <br /> OBSERVATIONS: <br /> Observed titanium jewlery 18ga 9/32". <br /> Unable to determine if jewlery meets HSC 119310 requirements. <br /> CORRECTIVE ACTION: <br /> Provide mill certificates to show that the jewlery meets the requirements outined in HSC 119310(b) Only jewelry made of ASTM <br /> F138, ISO 5832-1, and AISI 316L or AISI 316LVM implant grade stainless steel, solid 14-karat through 18-karat yellow <br /> or white gold, niobium, ASTM F 136 6A4V titanium, platinum, or other materials found to be equally biocompatible shall be <br /> placed in newly pierced skin. <br /> 33. HSC 119314 Workstation, surfaces, including chairs, armrests, etc. in good repair. <br /> OBSERVATION: <br /> Observed black tape on procedure bed located in the piercing room. <br /> CORRECTIVE ACTION: <br /> Repair/replace. <br /> All surfaces and objects in the procedure area, including chairs, armrests, tables, countertops, and trays, shall be in good repair <br /> and capable of being disinfected. <br /> All counter surfaces and service trays in the procedure area shall have a smooth, durable, and nonabsorbent finish. Stericycle is the sharps waste hauler. <br /> 3. 1 reviewed IPCP, training records and sharps waste hauler receipts. The last sharps pickup was 9/29/2025. <br /> 4. The IPCP was updated during inspection to include the ENBIO sterilizer time and temperature. Disinfectant information <br /> was also added for the additional disinfectatnt being used onsite(Opti-Cide Max). <br /> 5. Completed client consent forms were reviewed for 3/2/2026-4/2/2026. <br /> 6. Practitioner Jordan Junco is not longer working at the facility a of January 2026. Per HSC 119312 An owner of a body <br /> art facility shall notify the local enforcement agency in writing within 30 days of the resignation, termination, or new hire <br /> of a body art practitioner at the body art facility. <br /> 7. 1 received corrections for items#29 and#33 via email on 4/6/2026. <br /> Corrections due by April 28, 2026 <br /> Reinspection on/about: A reinspection fee of$179 per hour may be charged.
