Please complete the enclosed application and remit the appropriate fee at <br /> least 45 days prior to the planned date of occupancy of your facility. <br /> If there are any changes to the owner and/or operator information, a "Statement of Citizenship, <br /> Alienage and Immigration Status in Support of a Request for State Public Benefits" form must <br /> be completed and submitted to the Environmental Health Department prior to the issuance of a <br /> permit to operate. Please contact Lisa Vernengo, at (209) 468-9674 to obtain this form. <br /> If you do not intend to use the facility this year, please complete and return the enclosed <br /> "Certificate of Non-Operation." <br /> For your reference, the laws and regulations covering employee-housing facilities are found in <br /> the California Health and Safety Code, Division 13, Part 1, and in the California Code of <br /> Regulations, Title 25, Chapter 1, Subchapter 3. Please be advised that all employee housing <br /> accommodations, including single family homes and mobile homes, are subject to the <br /> Employee Housing Act, and must be inspected as part of the permit process. <br /> Please return the completed application and the applicable permit fees, or the Certification of <br /> Non-Operation, in the enclosed envelope.
