OBSERVATION: Designated operator reports were not signed within the required time frame. -The 12 months designated operator reports from 3/23/23 to 03/12/2024 were not signed and do not include owner/operator name and date. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. -------------------------------------------------------------------------------------------------------------------------------------------------------- 2). OBSERVATION:The designated operator failed to check and note all required items on the visual inspection reports.The missing information included the following from the inspection reports: -The 7/28/2023 and 4/7/23 DO reports section 5 follow up action for compliance issues stated in section 3 were left blank. -The DO reports did not include owner/operator name, signatures and dates. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class ll. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificate dated 1/12/2024 did not include the date of assuming responsibility as a facility employee and the Facility Employee Training Certificate dated 9/22/23 did not include the employees initial training dates . REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a repeat violation, Class ll.
