Required information was missing from several of the notifications, as <br /> noted below. <br /> -Veeder-Root vac sensor replacement on 3/15/23 <br /> Notification did not include which vacuum sensor was replaced, or the model#of the part used. <br /> -Cold start of the monitoring panel on 5/21/23 <br /> The notification states"performed sensor functionality of all liquid sensors". The alarm history printouts on site <br /> indicate that no vacuum sensors or leak detectors were tested after the cold start. <br /> -91 relay replacement on 10/1/23 <br /> -S4 vacuum sensor replacement on 11/8/23 <br /> Notification did not include model#of the part used. The alarm history printouts on site indicate that the vacuum <br /> sensor was not tested after the replacement. <br /> -87 relay replacement and 91 line PLLD sensor replacement on 11/24/23 <br /> Notification did not include the manufacturer or model#of the parts used.The alarm history printouts indicate that <br /> the only testing on 11/24/23 was the 87 leak detector,which does not appear related to the work in the notification. <br /> REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit <br /> obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by <br /> the EHD prior to beginning work.Work done using our permit option of"EHD Notification&On-site Paperwork" <br /> must comply with all conditions listed below. <br /> a) Provide 48-hour written notification to ust@sjgov.org for repair and testing. <br /> b)The component must be tested after repair/installation to verify functionality. <br /> c)The service company must provide the following documentation to the tank owner/operator <br /> for on-site review by an inspector during the annual UST inspection: <br /> 1) Description of part(s)installed (parts list, cut sheets, etc.) <br /> 2) Proof of current ICC and manufacturer certifications for individual performing the work <br /> 3)Testing documentation to verify functionality of the component after repair <br /> d)All repaired components must be tested within 30 days of repairs.Test results must be <br /> submitted to the EHD within 30 days of testing. <br /> If all conditions are not met, a"Failure to Obtain a Permit"penalty(an additional 100%of the full retrofit/repair permit <br /> fee)will apply. <br /> CORRECTIVE ACTION: Immediately obtain a permit from the EHD for each of the items listed above.
