This information was required to be submitted by <br /> 4/27/2024. This information was received on 5/3/2023, 6 days late. <br /> REGULATION GUIDANCE:An operator that receives an inspection report shall have 30 days to implement the <br /> corrections specified in the inspection report and comply with permit conditions. The corrective action shall include <br /> all of the recommendations made in the report. The operator shall submit a written response that includes a <br /> statement documenting corrective actions taken or proposing corrective actions which will be taken. <br /> CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is <br /> submitted and all corrections implemented within 30 days of receiving an inspection report. <br /> This is a repeat violation, Class II. <br /> 214 CCR 2712 Failure to comply with UST operating permit conditions. <br /> OBSERVATION: The facility is not in compliance with the written conditions of the operating permit. <br /> REGULATION GUIDANCE: The owner and operator of underground storage tank(UST)systems shall comply with <br /> all requirements of the permit issued for the operation of the UST system. <br /> CORRECTIVE ACTION: Immediately take all necessary actions to bring these UST systems into compliance. <br /> This is a Class II violation. <br /> 301 HSC 25292.1(a)Failure to operate UST system to prevent unauthorized releases including leaks,spills and overfills. <br /> OBSERVATIONS: <br /> -The facility uses wood as a lid for the vent sump.The wood cover was drilled into the vent sump. <br /> -The fluid in the vent sump reservoir was observed to be murky, and not the red/pink colored fluid the manufacturer, <br /> Western Fiberglass, provides.According to Western Fiberglass,their fluid becomes murky when another type of <br /> fluid from a different manufacturer is mixed with Western Fiberglass'glycol. Western Fiberglass stated that due to <br /> the mixing of another type of monitoring fluid, the interstitial space of the vent sump is not being properly monitored. <br /> REGULATION GUIDANCE: The underground storage tank system shall be operated to prevent unauthorized <br /> releases, including spills and overfills. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed and certified contractor to ensure the vent sump's <br /> interstitial is adequately monitored and the vent lid is installed according to the manufacturer's instructions. <br /> This is a repeat violation, Class II. <br /> FA0017819 PR0526335 SCO01 03/25/2024 <br /> EHD 23-01 Rev.12/06/2021 Page 5 of 8 Underground Storage Tank Program OIR <br /> 1868 E.
