Testing has not been completed and is currently 15 months past due. <br /> REGULATION GUIDANCE: Secondary containment testing is required to be performed once every 36 months. <br /> CORRECTIVE ACTION:Testing is scheduled for 9/6/2024. Complete the secondary containment testing and submit a copy of <br /> the test report within 30 days of the test. <br /> This is Repeated (Class 2) Violation. <br /> OVERALL NOTES AND COMMENTS <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the <br /> corrective actions that have been or will be taken for each violation, and any supporting paperwork, by October 2, 2024. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of having them <br /> sign. <br /> Starting July 1, 2024, all in-office CERS help will be provided at EHD hourly rate ($172).
