Page 4 of 5 <br />17. Local Enforcement Agency (LEA) Conditions: <br />1. The design and operation of this facility must comply with all applicable State Minimum Standards for solid waste handling as <br />specified in Title 14, California Code of Regulations. <br />2. The operator shall maintain a log of special/unusual occurrences. This log shall include, but is not limited to, fires, explosions, <br />the discharge and disposition of hazardous or un-permitted wastes, and significant injuries, accidents or property damage. Each <br />log entry shall be accompanied by a summary of any actions taken by the operator to mitigate the occurrence. The log shall be <br />available to site personnel and the EA at all times. <br />3. The operator shall comply with all waste discharge requirements, Clean-up and Abatement Orders, monitoring, remediation <br />schedules and related requirements of the local California Regional Water Quality Control Board (CRWQCB). Degradation of <br />water connected to this site shall be remediated in the manner specified y the CRWQCB. <br />4. Additional information concerning the design and operation of this facility must be furnished upon the request of the LEA. <br />5. The maximum daily vehicle traffic allowed the landfill and transfer station is 620 trips per day combined. The site shall not <br />receive more than 620 vehicles per day without first revising this permit. Vehicle numbers are to be tracked separately. This <br />requirement is already covered under Self Monitoring requirement k. <br />6. Forward Resource Recovery Facility must comply with the conditions of approval of Use Permit UP-00-07. <br />7. The LEA reserves the right to suspend or modify waste receiving and handling operations when deemed necessary due to an <br />emergency, a potential health hazard, or the creation of a public nuisance. <br />8. Any change that would cause the design or operation of the facility not to conform to the terms and conditions of this permit is <br />prohibited. Such a change may be considered a significant change, requiring a permit revision. In no case shall the operator <br />implement any change without first submitting a written notice of the proposed change, in the form of an RFI amendment, to the <br />LEA at least 150 days in advance of the change. <br />9. A change in operator for this site will require a new solid waste facility permit. The LEA shall be notified in writing of any <br />change in ownership or operator of the facility. <br />10. The facility shall meet the design and operational standards of the California Code of Regulations, Title 14 (CCR 14), Sections <br />17400-17564, and Title 14 (CCR 14), Sections 17850-17870. <br />11. The transfer station facility shall maintain and keep all stockpiles under 12 feet in height within the transfer building. <br />12. Physical Contaminants and refuse removed from feedstock, compost, or chipped and ground material shall be removed from the <br />site within 7 days and transported to a disposal facility. <br />13. All stockpiling of compostable organic material for a combined period of time greater than six months is considered disposal and <br />must be either removed from the site or processed for composting operations. <br />14. Wastewater shall not be allowed to discharge off the property or into any waterway except as allowed by NPDES permit. <br />15. All incoming waste shall be inspected for hazardous waste. If hazardous waste is found it shall be handled as hazardous waste in <br />accordance with all applicable State standards. <br />16. In accordance with Title 14 (CCR 14), Section 17867.5. Operators shall ensure that all personnel assigned to the operation shall <br />be trained in subjects pertinent to operations and maintenance, physical contaminants and hazardous materials recognition and <br />screening, with emphasis on odor impact management and emergency procedures. A record of such training shall be maintained <br />on the site. All personnel working with the load screening program shall be trained to recognize hazardous wastes, and shall be <br />trained for safety and evacuation procedure <br />17. This permit is subject to review by the LEA and may be modified, suspended, or revoked for sufficient cause following a hearing. <br />18. Wastes to be composted must go directly into composting operation and may not be stockpiled on site for more than seventy-two <br />(72) hours.
