Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />March 25, 2024 <br /> Facility Address: <br /> 2115 W YOSEMITE AVE, MANTECA <br /> Facility Name: <br /> CAGASOLINE INC <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 4 HSC 25508(a)(1) Failed to electronically submit the Business Activities or Owner/Operator Identification page. <br />OBSERVATION: The Business Owner/Operator Identification form is missing secondary emergency contact <br />information. <br />REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class II. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: Several car wash chemicals were found in the car wash area that have not been reported to <br />CERS. <br />REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br />facility's business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br />approval. <br />This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. The north orientation, emergency response equipment placed by the <br />dispensers, and loading areas were not documented on the site map. <br />REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br />evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br />present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map <br />requirements the governing body of the unified program agency finds necessary. <br />CORRECTIVE ACTION: The site map was updated and uploaded into the California Environmental Reporting <br />System (CERS) at the time of the inspection. <br />This was corrected on site. <br />This is a repeat violation, Class II. <br />FA0017819 PR0528756 SC001 03/25/2024 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 2 of 4 <br />1868 E.
