OBSERVATION: The Business Owner/Operator Identification form and Activities form was not submitted or complete. The Activities page does not have the Aboveground Petroleum Storage section marked as a yes and the Hazardous Waste section was left as a no, even though hazardous waste is generated at the site. REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. OBSERVATION: Three estimated 12,000 gallon tanks of diesel and four 25 pound tanks of CO2, an about 100 gallon tank of mixed car wash chemicals, and one about 50 pound tank of CO2 were found on site that have not been reported to CERS. REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. This is a Class II violation. 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. OBSERVATION:A site map was not completed and submitted as part of the business plan. The site map is missing the car wash,the Above Ground Storage tanks,the loading areas, and the location of the CO2 tanks.Additionally all hazardous material handling and loading areas need to be marked. REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map requirements the governing body of the unified program agency finds necessary. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval.
