OBSERVATION:According to the designated operator report dated 04/30/2024 the truck side dispensers were <br /> replaced and EHD was not notified. <br /> REGULATION GUIDANCE:The owner and operator of underground storage tank(UST)systems shall comply with all <br /> requirements of the permit issued for the operation of the UST system. <br /> CORRECTIVE ACTION: Provide a statement and immediately take all necessary actions to notify EHD within 48 <br /> hours for all repairs of UST and its components including emergency repairs. <br /> This is a repeat violation (Class 2) Violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br /> finished on 06/18/2024 and replaces the initial checklist. Complete and submit a copy of the Return to <br /> Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or <br /> will be taken for each violation, and any supporting paperwork, within 30 days of receiving the complete inspection <br /> report. <br /> *Very important Notes: <br /> -Approximately one gallon of liquid was observed in the bio diesel fill sump, satellite UDCs#22, 23, 24, 26, 27 and <br /> 29. <br /> -In CERS overfill prevention equipment method check as Audible/visual alarms and Shut-off valve.
