The waste from <br /> the referenced site has been approved for acceptance at our landfill as Class II waste. The approval has been based on the <br /> information provided in the Waste Profile Form and associated materials dated January 21, 1997 on behalf of T & T <br /> Trucking (Generator). Acceptance of the waste is subject to FORWARD, INC.'s receipt of this "Payment Terms and <br /> Conditions" form signed by you. Waste acceptance is also subject to regulatory requirements and the "Terms and <br /> Conditions"agreed to and signed by the Generator on the Waste Profile Form. <br /> The FORWARD, INC. landfill is located in Manteca, San Joaquin County, California and is a Class II/III landfill and <br /> treatment facility. FORWARD,INC. can accept non-hazardous waste as defined by the California Code of Regulations, <br /> Title 22, Section 66261.24, and in accordance with the Waste Discharge Requirements issued by the California Regional <br /> Water Quality Control Board, Central Valley Region dated March 2, 1994, Order no. 94-014. Class II units have the full <br /> protection of Subtitle D composite liner with a leachate collection system. <br /> The Landfill's operation hours for soil delivery are from 7:00 a.m. to 5:00 p.m. Monday through Friday. All other waste <br /> types are accepted from 7:00 a.m. to 3:00 p.m. Monday through Friday. Arrangements can be made to extend the <br /> Landfill's hours. Customer shall give FORWARD,INC. 24 hours notice prior to the delivery of any waste by calling the <br /> Landfill at(209)982-4298. Such notice shall include the anticipated number of trucks,total estimated volume of waste to <br /> be delivered and the description of any special equipment that shall be required for unloading and/or disposal. Customer <br /> shall conform to any reasonable routing requirements,safety protocol and dumping directions. <br /> Subject to the terms and conditions set forth herein,T&T Trucking(Customer)agrees to the following: <br /> 1. Customer shall pay disposal fee of$15.00 per ton for the Ongoing disposal of waste (collectively, the "Fee"). The <br /> Fee remains in effect provided the waste is delivered to the Landfill no later than . FORWARD, INC. reserves the <br /> right to increase charges without notice after that date,at is sole discretion. <br /> 2. The actual total amount of Fees charged for any given truckload of waste delivered to the Landfill shall be weight <br /> determined by certified weight tags,multiplied by the applicable Fee rate pursuant to Paragraph 1 above. <br /> 3. The Fee does not include transportation fees or transportation taxes. Customer is solely responsible for payment of <br /> any such fees and taxes. <br /> 4. The approval number for the waste is 570111.
