SEMINFOUR Site Hazard Analysis <br /> other traffic control devices are available. Project vehicles will be parked in designated parking areas that <br /> do not impede facility traffic or obstruct vision. Project field personnel should be aware that vehicles and <br /> heavy equipment may be operated in areas adjacent to the subject sites. <br /> Vehicle operators will check carefully for nearby traffic before proceeding at a cautious pace on <br /> roadways. Care should be taken to ensure that trucks,equipment, and materials are placed so obstruction <br /> of local traffic is minimized. During work activities,it may become necessary to move equipment in order <br /> to accommodate traffic and site activities. Refer to SMS 032 on work zone traffic control. <br /> 4.1.9 Excavation Safety Guidelines (SMS 013 &034) <br /> Underground Service Alert(USA) (1-800-227-2600)will be contacted 2 working days prior to any type <br /> of excavation to ensure underground facilities are located and avoided(8 CCR 1541(b)(2) and SMS 034). <br /> An open excavation/trench may be present. Personnel are prohibited from entering any trench or <br /> excavation that is not appropriately protected.The excavation contractor has to provide a trained <br /> competent person in accordance with OSHA and state specific regulations. All URS personnel working <br /> on the Site will have Competent Person Training. Monitoring will be conducted to detect hazardous <br /> atmospheres or other hazardous conditions within the excavation.A competent person is required to <br /> certify the excavation to be stable if it is less than 5 feet in depth.All shoring for excavations greater than <br /> 4 feet in depth is required to be certified by a competent person.Personnel that observe an excavation <br /> condition believed to be unsafe shall immediately advise the SM of the unsafe condition.All walkways <br /> over the excavation/trench must meet OSHA and state specific requirements regarding load limits, <br /> railings,and construction materials. See URS SMS 13 for additional information.Employees are <br /> responsible for their own safety including recognizing and avoiding excavation hazards (SMS 013). <br /> Personnel that observe an excavation condition believed to be unsafe shall advise the backhoe operator of <br /> the unsafe condition.URS 2011 California Excavation Permit must be available at the site during field <br /> activities (Appendix E). <br /> ' In the event of a cave-in or the excavation becomes a threat,all personnel will be evacuated from <br /> the trench until the hazardous situation is properly controlled or eliminated.In the event Site <br /> personnel become trapped in the excavation,call the Fire Department at(209)464-4650.This <br /> predetermined evacuation area will be discussed in the tailgate safety meeting. <br /> 4.1.10 Portable Ladders (SMS 028) <br /> All personnel are responsible for inspecting ladders prior to use and the proper use of stepladders and <br /> straight and extension ladders as described in SMS 028.
