CTI FOUR sue hazard Analysis <br /> 4.1.13 Heavy Equipment <br /> Only authorized personnel will operate equipment. Operators are responsible for the safe operation <br /> of equipment. Prior to the start of field operations, the SSO will ensure that every piece of equipment <br /> has been inspected, is in safe operating condition, and that a copy of the manufacturer's safety <br /> instructions are located at the site. The SSO must also see that each equipment operator has read <br /> and understood the manufacturer's safe operating instructions. Operator manuals and safety <br /> instructions,with identified equipment, must be maintained with each piece of equipment for the <br /> duration of the project. Operators will be responsible for inspecting equipment before each daily use; <br /> the inspections will be documented in the project files. Equipment will not be operated until all <br /> deficiencies have been corrected. <br /> 4.2 BIOLOGICAL HAZARDS (SMS 047) <br /> Biological hazards that may be encountered at the project work site could include spiders and <br /> biting/stinging insects. Personnel will be reminded prior to each day's activities to be aware of these <br /> hazards and to take the necessary precautions to avoid them by adhering to safe work practices(e.g., <br /> avoid reaching into covered or dark areas or picking up rocks and other objects). Individuals with specific <br /> allergies to insects should remember to note this fact to the SSO prior to the start of field activities.A first <br /> aid kit will be available to treat minor insect bites and stings. First aid procedures for minor insect bites <br /> and stings include: cold applications use of soothing lotions(e.g.,calamine),and for a bee sting,removal <br /> of the venom, stinger, and venom sac. If the bite or sting is from a poisonous spider or produces a severe <br /> reaction,implement the following procedures: Calm the victim and keep him/her from moving about, <br /> preferably in a prone position,and call 911. It is essential to get the victim to a hospital immediately. <br /> SMS 047(Biological Hazards)provides appropriate health and safety procedures for these hazards. <br /> 4.3 CHEMICAL HAZARDS <br /> This work is anticipated to be hazardous waste work as this site is known to have groundwater <br /> contamination from aluminum,arsenic,barium,iron,lead,manganese,and nickel.The list of chemical <br /> constituents is provided in Table 4-2 along with the information on health hazards,exposure limits, and <br /> respiratory protection procedures. <br /> The groundwater contains TDS, aluminum,arsenic,barium,iron,lead,manganese,and nickel at <br /> concentrations exceeding water quality goals;however,the task to destroy the wells will not have the <br /> TABLE 4-1 <br /> H:\WPROCESS\25532\WORLDENTERPRISE\WELL DESTRUCTION\ATTACHMENT C WORLD ENTERPRISES HSP TG EDITS.DOC\18-NOV-11\\4- <br />
