sEenINSIX Emergency Response Plan <br /> 6.3 NOTIFICATION (SMS 049 AND 065) <br /> All incidents require immediate review and completion of SMS 049 (Incident Reporting,Volume II). All <br /> injuries require immediate review and completion of SMS 065. In the event of an injury-related accident, <br /> hazardous substance release,damage to property,or emergency situations whether existing or imminent, <br /> the PM or designated field personnel must notify appropriate personnel as soon as possible but no later <br /> than within one to two hours. Personnel to be contacted are identified in the emergency telephone <br /> numbers. <br /> Accidents,injuries,non-injury incidents, and any other incident or noteworthy events will be immediately <br /> reported to the PM,and RSM on an Accident/Incident Report form within 24 hours of the incident. <br /> Circumstances of the accident/incident and preventive measures will be discussed with the SSO,PM, and <br /> project team field personnel prior to resuming regular activities during the next tailgate safety meeting. <br /> The SSO or PM will investigate cause(s) and recommend appropriate control measures.The RSM is <br /> responsible for reviewing the information and determining if further investigation or corrective measures <br /> are required. PM will also notify the appropriate state or federal agencies of any reportable spills or <br /> releases. <br /> Field personnel including subcontractors are responsible for reporting all work-related injuries,or <br /> incidents that may result in an injury,or illnesses as soon as possible to the SSO,RSM,PM, and <br /> appropriate URS supervisory personnel. Each individual project team member is responsible for <br /> documenting the incident and notifying their supervisor as soon as possible.The project manager and the <br /> RSM are responsible for notifying RadioShack and Cal-OSHA of any recordable injuries or illnesses to <br /> their employees,and maintaining health and safety files,including OSHA logs,training and medical <br /> surveillance certificates and records,and worker compensation files. Employee medical files,including <br /> records of work-related exposures,accidents/illnesses,are maintained by the project team member's <br /> occupational physician. <br /> 6.4 EXPOSURE/INJURY MEDICAL SURVEILLANCE (SMS 049 &065) <br /> Any employee who suffers an illness,suspected injury or chemical over-exposure is required to see a <br /> physician. Depending upon the extent and type of exposure,illness,or injury,it is critical to perform <br /> follow-up testing and/or accident investigation within 24 to 48 hours.The project team member's health <br /> and safety supervisory personnel will ensure that appropriate medical follow-up testing is conducted.The <br /> physician responsible for conducting the employee's medical surveillance examinations shall be notified <br /> and consulted to determine the type(s)of tests required to accurately monitor the employee. A worker <br /> may return to work only with the written approval of the attending physician.
