SECTIONFOUR Site Hazard Analysis <br /> 4.1.11 Fall Protection(SMS 040) <br /> Ensure that one or more of the fall protection/prevention systems outlined in SMS 040 is provided at all <br /> locations where fall hazards of 6 feet(2 meters)or greater exist.These locations include,but are not <br /> limited to,excavations,unprotected elevations,ladders,scaffolds,floor holes,wall openings,formwork, <br /> rebar tying,and all other locations and operations where potential fall hazards exist.Employees are <br /> responsible for their own safety including recognizing and avoiding fall hazards. <br /> 4.1.12 Utility Clearance Guidelines <br /> Underground Service Alert(USA) (1-800-227-260)will be contacted two working days before <br /> drilling(or any other type of excavation)to ensure underground facilities are located and avoided <br /> (8 CCR 1541(b)(2)and SMS 034). Heavy equipment safety and maintenance is the responsibility <br /> of the operator. URS employees are responsible for their own safety, including recognizing and <br /> avoiding heavy equipment hazards(SMS 056).URS employees that observe a heavy equipment <br /> condition believed to be unsafe will stop work and shall advise the operator of the unsafe <br /> condition. <br /> Work activities adjacent to overhead power lines will not be initiated until the safe clearance <br /> distance has been determined by the SSO.
