Date: 08/01/2026 <br /> Manufacturer training: NOV, 12/01/26 <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> NOTES <br /> Site Visit Findings: <br /> The following items could not be verified: <br /> Primary tank test for diesel,B20 diesel,and DEF <br /> Secondary piping test for product lines(diesel,B20,DEF),vapor recovery,vent lines,and chase piping <br /> Site Observations: <br /> Upon arrival,three pump shafts were observed lying on the ground adjacent to the B20 STP sump.Technician Ariel and his crew were inside both STP sumps, <br /> securing the manhole covers.According to Ariel,upon their arrival,both STP manholes and internal components had been disconnected within the STP sumps.The <br /> crew had to reassemble and reinstall all components before initiating the primary tank test. <br /> A walkthrough of the tank top area was conducted,and photos were taken of each metal manhole cover and the associated tank installation areas.A comparison <br /> was made between an existing photo of a rusted,single containment metal manhole cover and the manhole covers observed on site;however,no matching covers <br /> were identified. <br /> Meeting with Property Owner: <br /> Met with CJ Dhaliwa,the property owner,to address conflicting information provided by technicians regarding the tank repairs.According to the technicians,only the <br /> two diesel tanks had received any repair work. I informed CJ that my initial inspection report documented the presence of brine inside the DEF tank.Additional <br /> clarification was requested regarding the scope and completion status of the DEF tank repairs. <br /> Technician Inquiry: <br /> Technician Ariel was asked whether secondary piping tests would be conducted for the product lines,vapor recovery,vent lines,and chase piping.Ariel initially <br /> expressed uncertainty regarding the presence of chase piping.Upon further inspection,he confirmed that chase pipes were indeed installed on site. <br /> Testing Issue: <br /> Following the re-installation of the STP manholes and pump shafts,a pressure test was initiated on the B20 diesel tank.According to Ariel,a plug that had been <br /> installed during the original construction was now leaking air.Additionally,the crew was unable to remove the plug to inspect it for damage.Ariel was advised that <br /> the inspection would need to be rescheduled.He indicated he would contact Sean M.to report the issue and coordinate next steps. <br /> This facility is not approved to received fuel. <br /> This facility is not approved to operate. <br /> Additional inspections are required.Please see approval letter dated 6/14/2024 for a list of requirements.Submit inspection requires in writing to UST@sjgov.org <br /> with 48-hours advanced notice. <br /> To minimize person to person contact EHD is choosing to write the name of the person receiving the report instead of having them sign. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.If a reinspection is required,fees will be assessed at the current hourly rate. <br /> Received by: Name and Title: Emailed to Sean Murphy, Contractor <br /> EH Specialist: KRISTINASAETERN Phone: (209)616-3045 <br /> FA0024830 SR0083837 SC131 07/21/2025 <br /> EHD Rev.09/16/2020 Page 1 of 1 Service Request Inspection Report <br />
