A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and <br /> client, date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br /> sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br /> minimum of 90 days after use. HSC 119315(f)—The Everlasting Brows blades expired on August 10, 2021. Dispose <br /> blades at an approved sharps waste drop off location. Replace blades with unexpired blades and provide EHD with <br /> proof of purchase and a sterilization certificate. <br /> 13. Practitioner shall provide evidence of a completed, EHD approved, OSHA Bloodborne Pathogen Training consistent with <br /> section 119307.—A current Bloodborne Pathogen Training Certificate was not available for Gayle Witt or Kristen <br /> Patterson-Cranford. Provide EHD with a current BBP Certificate for both practitioners. <br /> 25. Disinfectant shall be used according to the manufacturer's instructions. HSC 119309(b)—Kristen Patterson-Cranford <br /> provided a wet contact time of 2 minutes for the disinfectant. Per the instructions on the label of the disinfectant,the <br /> wet contact time for HIV is 1 minute and the wet contact time for Hepatitis B is 5 minutes. Ensure the directions on the <br /> label are followed. <br /> 32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br /> The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br /> floor, from nail and hair activities and be separated from all business not related to body art. Approval of co-located body art <br /> facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br /> procedure area shall have lined waste containers. HSC 119314& 119317—There is a crack on the northern wall. Repair and <br /> paint the damaged area of the wall and provide EHD with photographic evidence of correction. <br /> 35. Operation and employee training records shall be maintained for 3 years and made available for inspection. A written <br /> Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated. HSC 119313—A training record was <br /> not available during the inspection. Practitioner(s)shall be trained on the IPCP once a year or when the IPCP is <br /> updated. Provide EHD a copy of the training record. <br /> 37. A body art facility shall not conduct business without a valid health permit. A person shall not perform body art without a valid <br /> registration and a permitted facility. A body art facility shall notify the LEA in writing within 30 days of resignation, termination of a <br /> practitioner. A new hire or apprentice must be registered with the EHD prior to performing body art at the facility. HSC 119312 & <br /> 119306—The facility health permit and Kristen Patterson-Cranford's health permit were not paid until after the <br /> inspection. Health permit fees are due prior to July 15Y of every year. Facility owner shall inform EHD within 30 days if <br /> there are any changes to operations. <br /> Correct all violations and provide evidence of corrections to the EHD <br /> by March 21, 2023. <br /> Reinspection on/about: A reinspection fee of$156 per hour may be charged.
