If you have any questions, please contact <br />Lynsey Sammons at (209) 616-3067 or Isammons@sjgov.org. <br />PACIFIC AVE, 6131 PACIFIC AVE, CERS ID: 10180905, PR0231223 <br />Violation #56 - 2030013 - DO <br />inspections completed at <br />least every <br />30 days <br />and meet all requirements <br />(TCR <br />12). <br />to <br />correct this issue <br />and remain in compliance. <br />Work orders from <br />the last 12 months <br />This violation was corrected ❑This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will betaken to correct violation: Met with owner of Eco-Chek (DO Inspectors) <br />to inform them of this deficiency and to remind them that all alarms need to be documented <br />on our monthly DO inspection report <br />that outline of all the work performed by a licensed technician on all fuel equipment. <br />Violation #59 - 2030001 - Records of release detection monitoring and maintenance properly maintained <br />(TCR 9d). <br />This violation was corrected ❑This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will betaken to correct violation: Business was purchased back in 08/2022 and no records <br />were provided for testing/compliance/DO reports. No reports available prior to that date. Since then, <br />we have maintained a strict electronic documentation and filing system which stores all DO reports, <br />annual testing, fuel equipment repair work orders and other required documentation available at <br />all times. <br />Violation #69 - 2030033 -A current UST monitoring plan has been submitted and is readily accessible at the <br />facility. <br />This violation was corrected 13 This <br />violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken orwill betaken to correct violation: UST Monitoring Plans are located on the tablets <br />under the folder "UST Monitoring Plans).
