The facility failed to conduct inspections and tests in <br />accordance with written procedures that the certifying engineer developed for the facility, including integrity tests, <br />signed by the appropriate supervisor or inspector. The inspection forms used onsite are not the forms included in the <br />Spill Prevention, Control, and Countermeasure (SPCC) Plan. <br />- The facility failed to conduct monthly inspections on the inspection form suggested to be used by the professional <br />engineer. The monthly inspection forms being used did not include testing overfill devices for functionality as stated in <br />the SPCC Plan 4.5.2 and 5.2.8 <br />- The annual inspections were not being conducted <br />REGULATION GUIDANCE: 112.7(e) Conduct inspections and tests required by this part in accordance with written <br />procedures that you or the certifying engineer develop for the facility. <br />CORRECTIVE ACTION: Immediately begin conducting all inspections and tests in accordance with the procedures <br />outlined in the SPCC Plan and begin using forms provided by the SPCC plan. Keep written inspection procedures and <br />records of inspections and testing signed by the appropriate supervisor/inspector for at least three years. Submit <br />proof of correction to the EHD. <br />This is (Minor) Violation. <br /> 57 <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 7/18/2024 <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br />having them sign. Starting September 1, 2018, all in-office CERS help will be provided at the EHD hourly rate <br />($162).
