OBSERVATION:The site map is incomplete. -Abbreviations are cut off from the bottom of the map -The symbol for the"Below Ground Tank"in the key(circle with 1 inside) does not match the label of the UST location on the map -The location of the CO2 tank is inaccurate -The location of the spill kit("Spill Control Equipment')is inaccurate -The symbol for the CO2 tank"Movable Pressurized Cylinders" in the key(4)does not match the symbol at the location of the CO2 tank on the map(3) REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map requirements the governing body of the unified program agency finds necessary. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a repeat violation (Class 2) Violation. Overall Inspection Comments: An inspection checklist was provided to the facility operator on the day of inspection. A complete report was finished on July 5, 2024 and replaces the initial checklist. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by August 4, 2024. To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of having them sign.
