(2) For the purpose of this subdivision, "substantial change"means any change in <br /> a facility that would inhibit immediate response during an emergency by either site personnel or emergency response <br /> personnel, or that could inhibit the handler's ability to comply with Section 25507 , change the operational <br /> knowledge of the facility, or impede implementation of the business plan. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is (Class 2)Violation. <br /> 9 CA Health and Safety Code 25508(a)(1) 1010003-Business Owner/Operator ID and Business Activities pages <br /> electronically submitted and are accurate. <br /> OBSERVATION:The Business Owner information on CERS is incorrect. Spoke to former owner Tony Romley and he <br /> stated the facility was sold to a new owner over 3 years ago, CERS still has former owner information. <br /> REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation (Class 2) Violation. <br /> 11 CA Health and Safety Code 25505(a)(2) 1010005-Site Map with all required content electronically submitted. <br /> OBSERVATION:The site map is incomplete. <br /> -Site map does not include emergency shut offs or Evacuation staging area. <br /> REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br /> evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br /> present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map <br /> requirements the governing body of the unified program agency finds necessary. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation (Class 2) Violation. <br /> 14 CA Health and Safety Code 25505(a)(4) 1020002-Provided initial and annual training and maintained training records <br /> for a minimum of three years. <br /> OBSERVATION:At time of inspection, training records were not available. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive <br /> initial and annual refresher training.All employees shall be trained in safety procedures in the event of a release or <br /> threatened release of a hazardous material, including, but not limited to,familiarity with the following plans and <br /> procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the <br /> unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential <br /> harm or damage to persons, property, or the environment.
