I understand that failure to comply with this agreement will result in <br /> further legal remedies. <br /> COMMENTS: Within 30 days of this office hearing, submit a Work Plan to the Department <br /> to address the following: <br /> 1.) Dispose of all tires from site to an approved Disposal Facility. Transport of 10 or more <br /> tires on public roads requires the use of a registered Waste Tire Hauler. Provide Waste <br /> Tire Manifests to the Department to document proper waste tire disposal. <br /> 2.) Correct all areas of subsidence and exposed waste with 12 inches of soil cover, <br /> compacted and graded, to prevent runoff of fill surfaces. <br /> 3.) Remove all solid waste that has accumulated on site and dispose of it at an approved <br /> disposal facility. Provide documentation to the Department for proper disposal of solid <br /> waste. <br /> Failure to provide a Work Plan to address above violations will result legal proceedings to <br /> include the issuance of Notice and Order. <br /> Proprietor's Sig re <br />
