HSC 119315 - Invoices and log kept for disposable, pre-sterilized equipment <br />OBSERVATION: <br />1. Some client logs are missing the artist name and date of the procedure. <br />2. Sterilization certificates were not available for XXX Tattoo needle lot numbers 24-01 and 23-08 <br />CORRECTIVE ACTION: When using disposable, single-use, pre-sterilized instruments, the following records must be <br />maintained: <br />1. Log of procedures (including names of the practitioner and client and the date of the procedure). Moving forward, ensure <br />to document all required information on records. <br />2. Written proof on company or laboratory letterhead showing that the pre-sterilized instruments have undergone a <br />sterilization process. Written proof shall clearly identify the instruments sterilized by name or item number and shall <br />identify the lot or batch number of the sterilizer run. Submit sterilization certificates to the Environmental Health <br />Department (EHD) for lot numbers 24-01 and 23-08. <br /> <br />#7. HSC 119314 - Sharps containers labeled, used, and disposed of appropriately <br />OBSERVATION: The facility could not provide sharps disposal records for the last 3 years. <br />CORRECTIVE ACTION: Documentation of proper disposal of sharps waste shall be maintained for three years and shall be <br />available for inspection. Submit sharps waste disposal records to the EHD for the last 3 years. If none are available, provide a <br />copy of the facility’s contract with Mediwaste. <br /> <br />#13. HSC 119307 - Bloodborne Pathogen training <br />OBSERVATION: All practitioners (Henry Coronilla, Jayla Sanders, and Jeremy Azevedo) have not renewed their bloodborne <br />pathogens training. Per Henry, he renewed his training and would email me his certificate. I have not received the certificate. <br />CORRECTIVE ACTION: Each person required to complete a Bloodborne Pathogens Exposure Control Training program <br />pursuant to this section shall annually complete a minimum of two hours of Bloodborne Pathogens Exposure Control Training. <br />Submit bloodborne pathogens training certificates to the EHD for Henry Coronilla, Jayla Sanders, and Jeremy Azevedo. <br /> <br />#18. HSC 119303 - Client records approved and available - Consent form and questionnaire <br />OBSERVATION: Some client records were missing client signature, client photo IDs and/or needle lot numbers. <br />CORRECTIVE ACTION: Moving forward, ensure to obtain all client signatures, maintain client photo IDs, and document needle <br />lot numbers for all procedures. <br /> <br />#32. HSC 119314 - Floors and walls clean and in good repair, adequate light <br />OBSERVATION: A section of the wall next to the procedure area sink has been removed. Per Henry, the facility is undergoing <br />plumbing repairs. <br />CORRECTIVE ACTION: A body art facility must have walls that are smooth, nonabsorbent, free of open holes, and washable. <br />Repair the wall next to the procedure area sink and submit photos to the EHD. <br /> <br />#33. HSC 119314 - Workstation, surfaces, including chairs, armrests, etc. in good repair <br />OBSERVATION: The front vacant station (cloth flag), Jaylas station (velvet box), and Jeremy’s station (cloth flag) had <br />absorbent materials. <br />CORRECTIVE ACTION: Corrected on site. Henry removed the items from the procedure area during the inspection. <br /> <br />
