This information was required to be submitted by 8/14/2019 and <br />was not submitted until 9/5/2019, 22 days late. <br />REGULATION GUIDANCE: 25404.1.2(c)(1) A person who receives a notice to comply detailing a minor violation shall <br />have not more than 30 days from the date of the notice to comply in which to correct any violation cited in the notice <br />to comply. Within five working days of correcting the violation, the person cited or an authorized representative shall <br />sign the notice to comply, certifying that any violation has been corrected, and return the notice to the UPA . <br />CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is <br />submitted within 30 days of receiving an inspection report. <br />This is (Minor) Violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 10/12/2024. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br />having them sign. Starting July 1, 2024, all in-office CERS help will be provided at the EHD hourly rate ($172).
