Lockout / Tag Out <br />3.1 Purpose <br /> <br />3.1.1 Establish the critical steps to for locking out hazardous sources of energy prior to <br />removing guards and cleaning, repairing, servicing, and adjusting equipment and <br />machinery capable of movement. All equipment shall be locked out to protect against <br />accidental or inadvertent operation when such operation could cause injury to <br />personnel. Do not attempt to operate any switch, valve, or other energy isolating <br />device bearing a lock. This procedure addresses Cal/OSHA requirements in the California <br />Code of Regulations (CCR) Title 8, §3314 Cleaning, Repairing, Servicing, and Adjusting <br />Prime Movers, Machinery and Equipment. Whenever the equipment or machinery is <br />located in a confined space, also reference Utilities’ Confined Space Policy & Procedures. <br />3.1.2 All equipment shall be locked out to protect against accidental or inadvertent <br />operation when such operation could cause injury to personnel. Do not attempt <br />to operate any switch, valve, or other energy isolating device bearing a lock. <br />3.1.3 Scope: <br /> This lockout policy applies to all Department personnel involved in the <br />process of locking out machinery or equipment or affected by such <br />lockouts. <br /> Whenever outside servicing personnel (vendors or contractors) are <br />engaged in activities covered by this Policy, this lockout procedure is <br />followed. <br /> Contractors are required to acknowledge receipt of a copy of this <br />procedure using Form A at the end of this procedure. <br /> These procedures do not apply to equipment connected by a plug that may <br />be removed from service as long as the individual cleaning, servicing, <br />repairing, or adjusting the equipment has the plug within their control. <br /> <br />3.2 Definitions <br />Authorized employee: An employee who locks or tags machines or equipment in order <br />to perform servicing or maintenance. <br />Affected employee: An employee who is required to use machines or equipment on <br />which servicing is performed under the Lockout / Tag-out standard or who performs <br />other job responsibilities in an area where such servicing is performed.
