Environmental Health Department <br />CP22 - HMBP Inspection Report <br />Date: <br />September 23, 2024 <br /> Facility Address: <br /> 3110 WAGNER HEIGHTS RD, STOCKTON <br /> Facility Name: <br /> MEADOWOOD A HEALTH & REHAB CTR <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br />information, and submit to the EHD for approval. <br />This is (Class 2) Violation. <br />CA Health and Safety Code 25508.1(a)-(f) 1010006 - Updated within 30 days of 100% increase or new haz; change of <br />address/owner/name; or change in ops. <br />OBSERVATION: During the inspection, Sergio Bustillos stated that Ryan Rocha has not worked at the facility for the <br />past 2 years. Ryan Rocha is listed as the Primary Emergency Contact. <br />- The business has not updated and submitted the information within 30 days. <br />REGULATION GUIDANCE: Within 30 days of one the following events, business shall electronically update the <br />information submitted to the California Environmental Reporting System (CERS): a) A 100 percent or more <br />increase in the quantity of a previously disclosed material. b) Any handling of a previously undisclosed hazardous <br />materials c) A change of business or facility address d ) Change business ownership e) Change of <br />business name f)(1) A substantial change in the handler 's operations occurs that requires modification to any <br />portion of the business plan. (2) For the purpose of this subdivision, “substantial change” means any change in <br />a facility that would inhibit immediate response during an emergency by either site personnel or emergency response <br />personnel, or that could inhibit the handler's ability to comply with Section 25507 , change the operational knowledge <br />of the facility, or impede implementation of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is (Class 2) Violation. <br /> 5 <br />CA Health and Safety Code 25505(a)(1) 1010004 - Complete and accurate Hazardous Materials Inventory information <br />electronically submitted. <br />OBSERVATION: The Hazardous Materials Inventory forms for Oxygen Gas were not complete. <br />- The amount of Oxygen listed in the inventory is inaccurate, it currently states that there is 20 cubic feet of the <br />material but while on site far more than 20 cubic feet was seen. <br />- Sergio Bustillos stated that the tank read that its capacity was 7,000 Liters and there were 2 tanks of the same size <br />in addition to approximately 40 small containers that were refilled by the 2 larger tanks. <br />REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part of <br />the business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is (Minor) Violation. <br /> 10 <br />CA Health and Safety Code 25505(a)(2) 1010005 - Site Map with all required content electronically submitted. <br />OBSERVATION: The site map is incomplete due to missing the following elements: <br />- North Orientation <br />- Loading areas of hazardous materials are not shown <br /> 11 <br />FA0010809 PR0521180 SC001 09/23/2024 <br />EHD Rev. 12/06/2021 CP22 - HMBP OIRPage 3 of 5 <br />1868 E.
