EMPLOYEE TRAINING: <br /> 40-hour Basic Training: <br /> Personnel assigned to the exclusion zone and contamination reduction zone must have completed the 40- <br /> hour basic health training required 29 CFR 1910.120 and have received 8-hour annual refresher training <br /> thereafter. <br /> First Aid/CPR: <br /> A minimum of one person per shift shall be currently First Aid/CPR certified. <br /> Confined Space Training: <br /> All persons required to enter confined spaces shall have confined space training. All confined space <br /> Entrants, Stand-by Attendants, and Entry Supervisors must be trained in the hazards of confined space <br /> work, the permit system being used and their specific duties and responsibilities as part of the entry team. <br /> MEDICAL SERVEILLANCE: <br /> Personnel assigned to field activities must participate in a medical surveillance program, in accordance with <br /> 29 CFR 1910.120 (f), with subsequent certification by an occupational physician of physical fitness and <br /> ability to perform assigned field work, including the ability to wear a respirator. <br /> AIR MONITORING INSTRUMENTS CALIBRATION AND RECORDKEEPIG: <br /> The multi-gas monitor instruments will be calibrated or performance checked and operated according to the <br /> manufacture's guidelines. Daily instrument readouts will be maintained by the Supervisor. Calibration <br /> records and records of exposure will be maintained in the project health and safety file. <br /> Confined Space Entry: <br /> Definition <br /> A confined space is defined by the concurrent existence of the following conditions: <br /> The existing ventilation is insufficient to remove air contamination and/or oxygen deficiency is less than <br /> 19.5% by volume, emergency access or egress for the removal of an injured person is difficult due to the <br /> location and/or size of the opening(s). A confined space is also defined as any space where human <br /> habitability is not possible. Examples of these spaces are silos, sumps, tanks, vats, vessels, boilers, ducts, <br /> compartments, sewers, ravines, pipelines, vaults, bins, tubs and pits. <br /> The following procedure sets forth the minimum requirements for safe entry, continued work in, and exit <br /> from a confined space. <br /> Procedures <br /> Written, understandable operating and rescue procedures shall be developed and provided to all employees <br /> involved. <br /> Written operating and rescue procedures will include, at a minimum, the following: <br /> • An approved site specific Health & Safety Plan. <br /> • An approved Confined Space Entry Permit. <br /> • A complete Standby Observer's checklist. <br />
