This is the split tank for this site. I put the UL number for <br />this tank in the box for #2 and #3. They are both 10’ diameter. The fluid levels on the tanks were Tank #1: 2”, <br />Tank #2-3: 2”, Tank #4: 6 1/2”, Tank #5: 4”. TANK INSTALLATION CHECKLIST <br />Missing on checklist: <br />- PAGE 37 <br />o Nominal Tank Diameter for TANK #3 <br />o Record fluid level in reservoir for double-wall wet tanks for TANK #3 <br />- PAGE 38 <br />o If applicable, record monitoring fluid level when backfilling & top slab installation are complete for <br />TANK #1 (20k), TANK #2, TANK #3, TANK #4 <br />2. FACILITY EMPLOYEE TRAINING CERTIFICATE <br /> <br />Gary, as discussed on the phone today, please see attached invoice for outstanding service request fees as well as <br />calculation for the CUPA Operating Permit fees. Please note that the outstanding fees for the service request is only able <br />to be generated up to May 2024 as we switched systems and does not yet have the ability to generate past May 2024. <br />An additional invoice for remainder of the service request fees to follow eventually when Accounting is able to generate <br />newer invoices. The Operating Permit fees are attached via an Excel worksheet calculation (again since Accounting is <br />unable to generate new invoices).
