All employees shall be trained in safety procedures in the event of a release or <br />threatened release of a hazardous material, including, but not limited to, familiarity with the following plans and <br />procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the <br />unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential <br />harm or damage to persons, property, or the environment. (C) Evacuation plans and procedures, including immediate <br />notice, for the business site. This training shall be documented electronically or by hard copy and shall be made <br />available for a minimum of three years. <br />CORRECTIVE ACTION: Immediately submit copies of the training records for the past 3 years to EHD. <br />This is (Class 2) Violation. <br />Overall Inspection Comments: <br />Routine HMBP Inspection <br />An inspection checklist was provided to the facility operator on the day of inspection, December 5, 2024. Item #11 <br />was added to the inspection report. A complete report was finished on December 5, 2024 and replaces the initial <br />checklist. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the complete inspection report., January 4, 2025 <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br />having them sign.
