The following is an itemized list of CP22- UST FULL violations that have not been <br /> addressed for FLAG CITY CHEVRON as of September 17, 2025. <br /> Open violations from April 29, 2025 inspection <br /> Violation#5-2030041 •A current facility plot plan is maintained and has been submitted. <br /> OBSERVATION:An accurate UST Monitoring Site Plan was not submitted. <br /> -The UST monitoring site plan states that the annular sensor is a VR 420,while on site a VR 409 annular sensor was <br /> found. <br /> REGULATION GUIDANCE:A site plan must be submitted identifying the locations where monitoring will be performed. <br /> The site plan must show the general layout of tanks and clearly identify locations of the following equipment, if <br /> installed: 1)monitoring panels;2)in-tank liquid level probes(if used for leak detection); 3)devices monitoring tank <br /> annular spaces or vault;4)devices monitoring product piping;5)devices monitoring fill piping;B)devices monitoring <br /> vent piping;7)devices monitoring vapor recovery piping; 8)devices monitoring vent/transition sumps;9)devices <br /> monitoring under-dispenser containment; 10)line leak detectors;and 11)devices monitoring any other secondary <br /> containment areas. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System(CERS)at <br /> http:ttcers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date). <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#12-2010009-Secondary containment test results submitted to the CUPA within 30 days after the <br /> test.. <br /> OBSERVATION: Secondary containment testing was performed on V812025 and a'test report was submitted on <br /> 3/312025,24 days late. <br /> REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION:Take all necessary precautions to ensure testing and submittal of test reports are performed <br /> in a timely manner. <br /> iZV This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct vioiaticn: � l'k" (� vJkt'- — �t <br /> �J t'�1 v'� • I kA 6k-t e-kq l0'`1iy <br /> Page 1 of 2 <br />
