OBSERVATION:An accurate UST Monitoring Site Plan was not submitted.The site map in CERS does not indicate location of annular sensors for diesel and 87 tank. REGULATION GUIDANCE:A site plan must be submitted identifying the locations where monitoring will be performed. The site plan must show the general layout of tanks and clearly identify locations of the following equipment, if installed: 1) monitoring panels; 2)in-tank liquid level probes(if used for leak detection); 3)devices monitoring tank annular spaces or vault;4)devices monitoring product piping; 5)devices monitoring fill piping; 6)devices monitoring vent piping; 7)devices monitoring vapor recovery piping; 8)devices monitoring vent/transition sumps; 9)devices monitoring under-dispenser containment; 10)line leak detectors; and 11)devices monitoring any other secondary containment areas. CORRECTIVE ACTION: This was corrected on site. A site map indicating location of annular sensors were uploaded to CERS during the inspection. This is a repeat violation (Class 2) Violation. 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all requirements(TCR 12). OBSERVATION:The designated operator failed to check and note the correct test dates for the Overfill Prevention Equipment testing on the designated operator inspection reports from 2/14/2024 through 7/3/2024.The incorrect date written was 9/27/2021.The correct date for the Overfill Prevention Equipment testing during that timeframe was 1/19/2024. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports.
