IS Outlook
RE:1501 N Jack Tone Rd-Notification
From Tara Velleux<Tara.Velleux@jfpetrogroup.com>
Date Thu 5/15/2025 11:18 AM
To Saetern,Kristina Gaen[EHD]<ksaetern@sjgov.org>
Cc Joey Cupp<joey.cupp@pilottravelcentem.com>;Miluska Cioffi<Miluska.Cioffi@pilottravelcenters.com>;Gregory Smith<Gregory.Smith@jfpetrogroup.com>;Robert Sills<Robert.Sills@jfpetrogroup.com>
Kristine,
I wanted to provide you with the notes from the work order that describe the resolution of the issues you witnessed while you were on site for the Monitor Certification. Everything was resolved on site
and positive shutdown was confirmed and all testing passed. Once test reports are QC'ed,we will submit through proper channels.
05113 Checked in with site manager Jay before beginning work. Here today to perform the Annual Monitoring Certification for 2025.Kristine from San Juaquin Environmental Health was on site to witness today's testing.
Techs began by testing hydrostatic testing of all spill buckets.Tank 1-Diesel spill bucket failed hydro test initially.Techs removed the fill adapter and EBW drain valve.Lots of debris was found at the bottom of the drain
valve and O-Ring causing the valve not to fully shut.Debris was removed and all components were reinstalled propedy.Retested the spill bucket for 1 hour and got passing results.1-hour extra time was spent
troubleshooting, repairing and retesting of the spill bucket. All spill buckets passed hydro testing after repairs.
While testing liquid sensors in the Auto UDC's we found that the 87 STP was not shutting down when the UDC sensors were in alarms.Checked programming and verified that all Auto UDC and sump liquid sensors
were programmed to shut down the 87 STP.The 87 STP Sump sensor was tested and confirmed positive shutdown was not operational.Further troubleshooting was needed.Hook Signals would turn on the 87 Relay
and STP,but VR would not interrupt the circuit when a sensor was in alarm.Believe that the issue may be due to a bad connection on the Output Relay Module board in#9 slot.Upon opening the Veeder-Root Door 1
found the wiring connections of the Output Relay Module for Slot 4 91 STP to be loose and appeared to have melted the plastic around the terminations.Found that due to Output Relay Module Slot Location for 87 STP
we needed to shut down the entire site to troubleshoot. The truck Diesel side was closed at that time to further investigate.Archived Veeder-Root programming and removed power from the TLS-350.Removed the
Output Relay Board and visually inspected the fuses.Fuses appeared to be in good condition.Reset the board into Output Relay Module slot 9 and re terminated all connections going into that board.Restored power to
the TLS-350 and we retested liquid sensors and positive shut down for the 87 STP.Positive shutdown was now operational for 87 STP but we found the 91 STP no longer turning on.This further delayed testing. With a
hook handle lifted, checked voltage coming into the Auto TLS-350 and found 0 volts when metering the Normally Open"NO"and Common"C"contact.Traced back the"C"contact wiring through the electrical conduit
and chase boxes to the Bio TLS-350 Output Relay Module Board.Confirmed 120volts at the"C"contact while the"NO"contact going to the Auto TLS-350 had 0 volts.Manually Tested the Output Relay for 91 STP while
testing with continuity at the"NO"and"C"contacts.Archived Bio Veeder-Root and removed power.Removed the Output Relay Board and found the fuse for the 91 STP position had blown.Replaced blown fuse with a
spare located on that board and reinstalled.Confirmed 91 STP issue to be due to the loose wiring causing the Output Relay fuse to blow in the TLS-350 Bio Veeder Root. With power restored to both VR's I confirmed
voltage for 91 STP was at 120 and STP now operable.Approximately 3 extra hours were spent troubleshooting the 87 Positive Shutdown and 91 STP issues.
After issues were addressed,we continued with testing. All liquid sensors, hydro sensors, vacuum sensors, sensors out,and positive shutdown were confirmed operational and passed testing.
Tested total of 6 Line Leak Detectors.Tank 2-Truck Diesel VMI 99-LD2000 failed to detect the 3.00 GPH leak rate initially.Made one adjustment to the MILD and retested.Testing passed after adjustment was made.0.5
hours extra time was spent adjusting and retesting the MLD.All Line Leak Detectors passed the test.
"L 11:Sensor Out"for 91 Annular Sensor on AutolTruck TLS-350 and"s2:Vacuum Warning"for RV 19,20 Prod vacuum zone on the Bio TLS-350 was active at time of arrival.Site has had recent open work orders for both
alarms that may need follow up.While on site performing testing,we tested sensors and confirmed they were operational at this time.Alarms self-cleared but if they return site is to create service ticket to further
investigate. All alarms caused while on site had cleared after work was completed.Sensors confirmed in proper locations and locked.Environmental Checklist Completed with on duty site manager.
Thank you,
Tara
Tara Velleux
CDCompliance Lead
9595 Lucas Ranch Rd.,#100,Rancho Cucamonga CA 91730
888-972-7581 1 Q 617-817-5319
jt?NESCUVEYr,RgI,R,INC.
