SAN JOAQUIN AMENDED Environmental Health Department <br /> COUNTY— 1/6/2026 <br /> CP26 - UST FULL Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CHEVRON USA#201761* 11103 S MAIN ST, MANTECA January 5, 2026 <br /> 5 23 CCR 16 2613(a)(6) and (b), 2650(a), (d), and (e) 2011013 - Facility has submitted a complete and accurate UST <br /> Response Plan <br /> OBSERVATION: An approved copy of the response plan was not available for inspection. <br /> -The emergency contact information of the Responsible person Diane Province, has not been provided. <br /> REGULATION GUIDANCE: Owners or operators must submit a response plan to the Unified Program Agency which <br /> demonstrates, to the satisfaction of the Unified Program Agency, that any unauthorized release will be removed from the <br /> secondary containment as soon as practical. This must be within a time consistent with the ability of the secondary <br /> containment to contain the hazardous substance but must not exceed 30 days. The response plan must include, but is <br /> not limited to, the following: <br /> (1)A description of the proposed methods and equipment to be used for removing and properly disposing of any <br /> hazardous substances, including the location and availability of the required equipment if not permanently on site, and an <br /> equipment maintenance schedule for the equipment located on site. <br /> (2) For methods of monitoring where the presence of the hazardous substance in the interstitial space cannot be <br /> determined directly, for example, where liquid level measurements are used as the basis for determination, the response <br /> plan must specify the proposed method(s)for determining the presence or absence of the hazardous substance if the <br /> indirect method indicates a possible unauthorized release of hazardous substance. <br /> (3)The name(s), title(s) and emergency contact information of the person(s) responsible for authorizing any work <br /> necessary under the response plan or, if applicable, identify that there is a continuously staffed emergency operations <br /> center authorized to coordinate such a response and provide a 24-hour phone number for that center. <br /> CORRECTIVE ACTION: Submit a complete and accurate UST Response Plan. <br /> This is a (Minor)Violation. <br /> 7 23 CCR 16 2613(a)(8) and (b), 2630(b) 2011004 - Owner/Operator submitted DO form within 30 days of UST install or <br /> DO change (TCR 10) <br /> OBSERVATION: The name of the DO Trent Heinrich is not identical to that individual's name as listed on the individual's <br /> International Code Council UST System Operator certificate. <br /> REGULATION GUIDANCE: 2630(b)Within 30 days of installing an underground storage tank system or any change to <br /> the designated underground storage tank operator(s), the owner or operator must submit to the Unified Program Agency <br /> information identifying the designated underground storage tank operator(s)for the facility. The name of each designated <br /> underground storage tank operator listed in the California Environmental Reporting System must be identical to that <br /> individual's name as listed on the individual's International Code Council UST System Operator certificate. <br /> CORRECTIVE ACTION: Submit a current Designated Underground Storage Tank Operator Identification Form. <br /> This is a (Minor)Violation. <br /> 40 23 CCR 16 2613(e) 2011002 - Installation, repair, lining, integrity, calibration, and release records maintained for life of <br /> UST <br /> OBSERVATION: Records of installation, repairs, linings, integrity certification, tank calibration charts, and/or <br /> unauthorized release reports were not kept onsite, or offsite if approved by the UPA, for the life of the UST. <br /> -The following were not available for review at the time of inspection: installation test results, manufacturer checklists <br /> and as built drawings. <br /> REGULATION GUIDANCE: (e)The following records must be maintained by the owner or operator on site, or off site at <br /> a readily accessible location if approved by the Unified Program Agency, for the life of the underground storage tank <br /> system: (1) Installation records including, but not limited to: (A) Installation test results; (B) Manufacturer checklists and <br /> manuals; and (C)As-built drawings. <br /> CORRECTIVE ACTION: Immediately locate and maintain these records onsite and submit copies to the UPA within 30 <br /> days. <br /> This is a (Minor)Violation. <br /> EHD-CUPA Inspection Report IS03 Page 5 of 6 FA0004521 PR0232587 4595502 1/6/2026 <br /> a <br /> CP26-UST FULL g <br /> 1868 E.
