The following is an itemized list of CP22 - HMBP violations that have not been <br />addressed for STKN MUD WATER WELL #20 as of February 03, 2025. <br />Open violations from May 10, 2024 inspection <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #10 - 1010004 - Complete and accurate Hazardous Materials Inventory information electronically <br />submitted. <br />BSERVATION: 55 gal drum of used oil found on site that have not been reported to CERS. <br />REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br />facility's business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #11 - 1010005 - Site Map with all required content electronically submitted. <br />OBSERVATION: The site map is incomplete, due to the following missing elements: <br />- Map indicates chlorine is on site, chlorine was not on site and not included in the inventory. <br />- Map does not indicate location of eyewash station/shower. <br />- Map does not indicate Loading zone for hazardous materials. <br />REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br />evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br />present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs , as well as additional map <br />requirements the governing body of the unified program agency finds necessary. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> <br />Page 2 of 3
