HSC 119315-Invoices and log kept for disposable, pre-sterilized equipment <br /> OBSERVATIONS: <br /> A sterilization certificate was not available for the TatSoul needle cartridges with the lot#240402. <br /> CORRECTIVE ACTIONS: <br /> A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and client, <br /> date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br /> sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br /> minimum of 90 days after use. Provide EHD with a sterilization certificate for the TatSoul needle cartridges mentioned above. <br /> Correct all violations and provide EHD with evidence of correction <br /> by June 20, 2024. <br /> Notes: <br /> 1. Inspection report emailed to owner, Julia Quintana, at ireneeauintanatc7i.gmail.com. <br /> 2. Operator is using the Voluta app for client consent forms and medical questionaires. All of the requirements from Form B are <br /> included on the forms. <br /> 3. Stericycle is used for sharps waste disposal.
