HSC 119315 -Invoices and log kept for disposable, pre-sterilized equipment <br /> OBSERVATIONS: <br /> A sterilization certificate was not provided for the TatSoul cartridges with the lot#240902-2. <br /> CORRECTIVE ACTIONS: <br /> A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and client, <br /> date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br /> sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br /> minimum of 90 days after use. Provide EHD with a sterilization certificate for the cartridges mentioned above. <br /> Correct all violations and provide evidence of correction to the EHD <br /> by March 26, 2025. <br /> Notes: <br /> 1. Inspection report emailed to facility owner, Julia Quintana, at ireneequintana(a�gmail.com. <br /> 2. Facility owner is using Voluta app for client records. Client records were printed for the inspection. <br /> 3. Stericycle is utilized for sharps waste disposal. The last pick up was on May 9, 2024. <br /> 4. Ensure there is no eating or drinking in the procedure room. <br /> 5.Above Training is no longer on the approved course list for Bloodborne Pathogen Training.
