California Street <br />Stockton, CA 95203 <br />It has come to our attention that your mobile food facility (MFF) is no longer operating from <br />an approved commissary. The California Retail Food Code Section 114295 requires that <br />you operate out of an approved commissary to service your vehicle at least once every day. <br />Your permit for the above mentioned MFF is suspended. If you intend to continue the <br />operation of your MFF and want to preclude the revocation of your permit, you must submit <br />the enclosed "Commissary Agreement" for approval within 15 days from the date of this <br />letter. <br />If you have any questions regarding this matter, please contact Lydia Baker, Senior <br />Registered Environmental Health Specialist, at (209) 616-3046. <br />Jeff Carruesco, REHS, RDI <br />Program Coordinator <br />JC/MF/jc
