The medical waste management plan (MWMP)does not contain all information listed in HSC <br /> 117960. <br /> -MWMP shall contain all information listed in HSC 117960. The Medical Waste Management Plan requires updating with <br /> facility specific information. Submit an updated Medical Waste Management Plan to the EHD by 3/17/20. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and shipping and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, and <br /> shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. HSC117975(a). No <br /> evidence of medical waste pick-ups for the periods of 5/22/2019 to 5/31/2019,6/13/2019 to 6130/2019,and for 2/13/2019 <br /> to 2/20/2019. Submit documentation to demonstrate compliance with the required 7-day pick-up frequency for the <br /> periods indicated to the EHD by 3/1712020. <br /> 23. Biohazard bag does not meet the standard set forth in HSC 117630. <br /> - Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear-resistant. HSC 117630 <br /> The biohazard waste bags in use in the facility only meet the ASTM D-1709 standard,and can only be used to collect <br /> waste in the facility. The bags must be placed in a biohazard container lined with a bag that meets the ASTM D1709 <br /> and the ASTM D1922 standard prior to shipping the waste offsite. Submit evidence of the purchase of biohazard waste <br /> bags that meet both the ASTM D1709 and ASTM D1922 standards to the EHD by 3/17/2020. <br /> 24. Pharmaceutical waste is not segregated for storage. <br /> - Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a container or <br /> secondary container labeled with the words"HIGH HEAT OR INCINERATION ONLY"on the lid and sides, so as to be visible <br /> from any lateral direction. HSC 118275(a)(6)(B). The facility collects pharmaceutical waste in five-gallon buckets with a <br /> single label on the top that states`Incineration Waste Only." Submit evidence of correction to the EHD by 3/17120. <br /> 25. Biohazard bag is not tied to prevent leakage or expulsion of content. <br /> ()The biohazard bag that is used to collect medical waste within a facility shall be manufacturer certified to meet the ASTM <br /> 1709 dart drop test, provided that when the bag is prepared for transport offsite, it is placed into a USDOT-approved container <br /> lined with a biohazard bag that is ASTM D1709 and ASTM D1922 certified. HSC 117630 <br /> The facility places sharps containers and ASTM D-1709 certified biohazard bags in unlined biohazard waste containers. <br /> Submit evidence of purchase of biohazard bags for lining the biohazard waste containers used to ship medical waste <br /> offsite that are compliant with both the ASTM D-1709 and the ASTM D-1922 standards to the EHD by 3/17/2020. Submit <br /> photographic evidence of biohazard waste containers lined with compliant bags to the EHD by 3/17/2020. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
