A test report has not been <br /> submitted to the EHD or the California Environmental Reporting System (CERS). <br /> REGULATION GUIDANCE:A copy of the inspection report form must be submitted within 30 days of the test. <br /> CORRECTIVE ACTION: Immediately provide this report to the EHD. <br /> This is (Minor)Violation. <br /> 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all <br /> requirements(TCR 12). <br /> OBSERVATION:The designated operator failed to check and note the correct test dates for the secondary <br /> containment test on the designated operator inspection reports.The correct date for secondary containment testing is <br /> 10/22/2024. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br /> required information on the reports. <br /> OBSERVATION:The designated operator failed to check and note all required items on the visual inspection reports. <br /> The incorrect and missing information included the following from the inspection reports: <br /> - DO certification is expired <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing <br /> and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all <br /> of the required information on the reports. <br /> This is a repeat violation (Class 2) Violation. <br /> 59 CA Code of Regulations 2712(b)(1-2)2030001 - Records of release detection monitoring and maintenance properly <br /> maintained (TCR 9d). <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br /> -The 2024 and 2023 monitoring system certification test results were not observed on-site. <br /> -The 2024 secondary containment test results were not observed on-site. <br /> -The 2024 overfill prevention equipment test results were not observed on-site. <br /> REGULATION GUIDANCE:These records shall be maintained on site for at least three years.
