OBSERVATION: The designated operator failed to check and note the correct test dates for the Overfill Prevention <br /> Testing on the designated operator inspection reports, the correct date is 7/25/2025. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Owner was informed that the incorrect date was being used in the DO reports, no further <br /> action needed. <br /> Observation#2 was corrected on site <br /> ----------------------------------------------------------------------------------------------------------- <br /> 2. OBSERVATION:The designated operator failed to check and note all required items on the visual inspection <br /> reports. The incorrect and missing information included the following from the inspection reports: <br /> -Incorrect facility address used for multiple DO Reports <br /> -Section 4 on the 5/7/2025 report did not have a date for when the report was given to the owner <br /> -CERS ID was not included on any DO reports <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing <br /> and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Owner was informed that not all required items were being included in the visual inspection <br /> reports, no further action needed. <br /> Observation#2 was corrected on site. <br /> ------------------------------------------------------------------------------------------- <br /> 3.
