Hazelton Avenue <br /> Stockton, CA 95205 <br /> Dear Mr. Escotto: <br /> The purpose of this letter is to request an emergency one-time extension be granted <br /> regarding the Medical Waste Removal Services for Deuel Vocational Institution (DVI) <br /> located at 23500 Kasson Road,Tracy,CA 95376. <br /> DVI is currently contracted with Converge Medical Solutions located at 1430 Koll Circle, <br /> Suite 103, San Jose, CA 95112. The telephone number for this contractor is <br /> (408)256-6663—Direct Number,or(408)436-2000,extension 301—Main Number. <br /> The Medical Waste was scheduled to be removed from the institution on Wednesday, <br /> September 4, 2013. Due to the Labor Day Holiday, the vendor failed to arrive as <br /> scheduled for service. Additionally, the vendor failed to give the institution prior <br /> notification, thus creating the time between services to go beyond the seven day <br /> maximum as stipulated per the Medical Waste Management Act, Section 118280(d)A, <br /> Containment and Storage.
