A large quantity generator required to register with the Enforcement agency shall file with the Enforcement agency <br />a medical waste management plan, on forms prescribed by the enforcement agency, if provided. The plans shall <br />contain but are not limited to (h) an emergency action plan complying with regulations adopted by the department. HSC <br />117960. <br />OBSERVATIONS: <br />1. Number 9 on the Medical Waste Management Plan Provided by EHD states “Describe the Emergency Action Plan that <br />the facility will follow to ensure the proper disposal of medical waste in the event of equipment breakdowns, natural <br />disasters, or other occurrences”. Plan states “See Policy 4-03-01CA”. No observed plan for proper disposal of medical <br />waste during emergencies was noted within the references policy. <br />2. Operator certification on the Medical Waste Management Plan had the old operator’s information. <br />CORRECTIONS: <br />1. Provide an Emergency Action Plan for proper disposal of medical waste during emergencies. <br />2. Update operator certification with signature submit a copy of the updated Medical Waste Management Plan to EHD. <br />13. Large quantity medical waste generators are required to maintain, for a minimum of two years, treatment and <br />tracking records for all untreated medical waste shipped offsite for treatment. HSC 117975(a) <br />OBSERVATIONS: <br />1. Tracking and treatment record dated June 18, 2025, did not have a generator name, signature or date. Carbon copy <br />writing was very light and not legible. <br />CORRECTIVE ACTION: <br />1. Please provide a copy of the tracking and treatment record with the necessary information stated above or provide in- <br />house training to staff members to ensure that the tracking and treatment records are being maintained properly and are <br />being completed in full. <br />23. The biohazard bag that is used to collect medical waste within a facility shall be, provided that when the bag is <br />prepared for transport offsite, placed into a USDOT-approved container lined with a biohazard bag. <br />OBSERVATIONS: <br />1. Bins that were in the designated accumulation area were not lined with biohazard bags. <br />CORRECTIVE ACTIONS: <br />1. Line containers in designated accumulation area with proper fitting biohazard bags. <br />24. To containerize or store medical waste, at the point of generation and while collected in that room, a person shall <br />have medical waste be contained separately from other waste at the point of origin in the producing facility. HSC <br />118275 <br />OBSERVATIONS: <br />1. Mixed waste was found to be within the sharps containers that were currently being used on the dialysis floor. <br />CORRECTIVE ACTIONS: <br />1. Please provide adequate training for the staff members of the facility to mitigate mixing other waste streams into medical <br />waste containers. <br />Corrections due to EHD by 7/24/25.
