SAN JOAQUIN <br />r_ COUNTY— <br />Environmental Health Department <br />WASTE DISCHARGE/SPILL RESPONSE <br />I0USIA ILO WOXMIAM <br />Responsible Party: PG&E Date: Aug 20, 2018 <br />RP Mailing Address: PO BOX 7640 Record ID#: COPFX21_5M <br />SAN FRANCISCO, CA 94120- Program Element: 2546 <br />Spill Location: 350 N UNION ST, MANTECA Release/Spill Response (excluding Joint Team) <br />NATURE OF WASTE DISCHARGEMASTE RELEASE: <br />Approximately 40 gallons of insulating oil, PCB unknown, was released from a dirt bottom vault transformer due to <br />equipment failure. <br />OBSERVATIONS: <br />Arrived on site and met with Bo McKeirnan of PG&E. Excavation of the impacted soil inside the dirt bottom vault had <br />already been completed by PG&E. Site will be leveled out and then backfilled. A sample of the insulating oil will be sent in <br />for PCB analytical. Spill area covered an approximately area of a three foot diameter circle. No waterways or storm drains <br />were impacted. PG&E to provide San Joaquin County Environmental Health Department (SJC EHD) with PCB analyticals <br />and final disposal records for the waste generated. <br />TIONS: <br />Discharge of any waste, including sewage <br />(CA Health and Safety Code 5411) <br />Discharge of waste or sewage to water or within 150 ft. of waterway <br />(SJC Ordinance Code Sections 9-1125.3, 9-1125.4) <br />Improper disposal of a hazardous waste <br />(CA Health and Safety Code 25189.5) <br />Permitting or Maintaining a Nuisance <br />(SJC Ordinance Code Section 8-5100) <br />11 Other Violation <br />RRECTIVE ACTIONS / ORDER: <br />• Investigate and abate contamination under the oversite of the appropriate lead agency. <br />• Submit copies of hazardous waste manifests/disposal records to the EHD. <br />• Other Correction <br />Submit PCB analytical to SJC EHD. <br />COST RECOVERY: All EHD time associated with this Spill Response is billed to the responsible party at the current hourly <br />rate.
