HSC 119313—A body art facility shall mainatain and follow a written Infection Prevention and Control Plan, <br /> provided by the owner or established by the practitioners, specifying the procedures to achieve compliance with each <br /> applicable requirement of this chapter. <br /> OBSERVATIONS: <br /> 1. Infection prevention control plan did not include the new sharps waste hauler information. —CORRECTED ON SITE. <br /> 2. Infectction prevention control plan did not have the practitioner signature at the end of the plan. —CORRECTED ON <br /> SITE. <br /> Notes: <br /> 1. Report is emailed to ski nshadesstudio(cDoutlook.com <br /> 2. Client consent forms were reviewed for the past year. Client consent forms did not contain the ID scans. Practitioner <br /> was able to pull up all IDs from the client consent form on her phone. <br /> 3. Facilty opened in 2024. Only 1 sharps waste manifest was provided. Sharps waste manifest dated 2/15/25. Noted to <br /> operator that sharps waste needs to be pick up when container is at fill line, or at least 1 time per year. <br /> 4. Sterilization certificates for Mast Pro needles (1009M and 1003RL with lot number 2025-10-01), Mast Pro needles <br /> (1001 RL with lot number 2025-11-01), Cheyenne (5 Shader, 0.30 with lot number 2024-14-036)were all observed on <br /> site. <br /> 5. Updates were made to Infection Prevention Control. It was noted to operator the new version of the Infection Prevention <br /> and Control Plan needs to be submitted to EHD. <br /> 6. Facility and practitioner permits are paid, but permits have not been mailed yet. Once received, operator is to send proof <br /> that permit is posted within the facility. <br /> Reinspection on/about: A reinspection fee of$179 per hour may be charged.
