The following is an itemized list of CP22 - HMBP violations that have not been <br /> addressed for CHIPOTLE MEXICAN GRILL#4681 as of August 06, 2025. <br /> Open violations from April 16, 2025 inspection <br /> Violation#3-1010017-Business plan readily available to site personnel responsible for emergency <br /> response or training. <br /> OBSERVATION: Facility could not provide the Emergency Response Plan and the inventory information upon request. <br /> Also, Employees do not have access to the Emergency Response Plan and the inventory information on-site. <br /> REGULATION GUIDANCE:The emergency response plans and procedures, the inventory of information required by <br /> this article, and the site map required by this section shall be readily available to personnel of the business or the <br /> unified program facility with responsibilities for emergency response or training pursuant to this section. <br /> CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#11 -1010005-Site Map with all required content electronically submitted. <br /> OBSERVATION: The site map is incomplete, due to the following: <br /> -The location of the CO2 tank on the site map is inaccurate. The CO2 was found located north of the soda syrups. <br /> REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br /> evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br /> present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map <br /> requirements the governing body of the unified program agency finds necessary. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 1 of 2 <br />
