The following is an itemized list of CP22 - HMBP violations that have not been <br /> addressed for BURGER IM as of August 06, 2025. <br /> Open violations from March 31, 2025 inspection <br /> Violation #14 - 1020002 - Provided initial and annual training and maintained training records for a <br /> minimum of three years. <br /> OBSERVATION. Employees have never been trained in safety procedures in the event of a release or threatened <br /> release of a hazardous material, <br /> - Rajesh Pathak was unaware that this training was required, he was provided an overview of the type of training that is <br /> required. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive <br /> initial and annual refresher training. All employees shall be trained in safety procedures in the event of a release or <br /> threatened release of a hazardous material, including, but not limited to, familiarity with the following plans and <br /> procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the <br /> unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential <br /> harm or damage to persons, property, or the environment. (C) Evacuation plans and procedures, including immediate <br /> notice, for the business site. This training shall be documented electronically or by hard copy and shall be made <br /> available for a minimum of three years. <br /> CORRECTIVE ACTION, Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training r rds to the EHD. <br /> This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: Oljr'j�t rAl <br /> Al �� AJA <br /> � <br />
