The document provided by the facility is for a "proposed" project <br /> and does not show materials of all conveyances and containers. <br /> -The facility failed to provide documentation demonstrating that the waste wash water conveyances and containers are <br /> visually inspected twice per year to ensure that waste wash water is not leaking into the underlying soil as required. The <br /> facility provided documentation demonstrating that the visual inspections are conducted once per year. <br /> -The visual inspection records submitted by the facility show that wastewater pit#1 failed hydrostatic testing on 9/28/24 <br /> and 3/1/25.
