The document provided by the facility is for a "proposed" project and does not show materials of all conveyances and containers. -The facility failed to provide documentation demonstrating that the waste wash water conveyances and containers are visually inspected twice per year to ensure that waste wash water is not leaking into the underlying soil as required. The facility provided documentation demonstrating that the visual inspections are conducted once per year. -The visual inspection records submitted by the facility show that wastewater pit#1 failed hydrostatic testing on 9/28/24 and 3/1/25. On 9/28/24, the pit lost 12 inches within 24 hours and then an additional 4.5 inches, totaling 16.5 inches lost. On 3/1/25, the pit lost 13 inches within 24 hours and then an additional 4.5 inches, totaling 17.5 inches lost. -The contingency plan submitted by the facility is limited to spill response and does not adequately address procedures for handling both onsite and offsite emergencies for the materials.
